When you submit a referral we collect: referrer name, relationship to client, contact phone and email, client name, date of birth (optional), address, reason for referral, current difficulties, relevant medical conditions (if provided), awareness of referral, preferred contact method, and your consent confirmations.

Why we collect it

We use this information solely to respond to your referral, assess clinical needs, deliver Occupational Therapy services, and meet our professional and legal obligations.

How we store it

Referral data is stored securely in our encrypted database with access restricted to authorised personnel only. We retain clinical records in line with HCPC and professional record-keeping guidance.

Your rights
